Checkout History/My Lists Request

If you enabled checkout history and/or created lists in the old MPPL catalog, please fill out this form to receive copies. Please keep in mind that your account needed to have this feature activated to retrieve the information, as checkout history was not automatically saved.

If you provide your e-mail address, we will send your history and/or lists electronically. If you do not provide an e-mail address, library staff will call you to arrange pickup or delivery of your history and/or lists.

Please allow up to 7 business days to generate your history and/or lists.
