1. I, the undersigned patron, understand and agree that I am responsible for reimbursing the library for any damage to or the loss of devices or any other contents contained in Library of Things items or kits.

I voluntarily release, discharge, waive, and hold harmless the Mount Prospect Public Library and its employees and the Board of Trustees from any loss, damage, or injury to persons or property arising from Library of Things equipment. In no event shall the Library be liable for indirect or consequential damages.

2. Some Library of Things items and kits specify that they must be returned to either the Patron Services desk at the Main Library located at 10 S. Emerson Street, Mount Prospect, IL or the South Branch desk at the South Branch Library located at 1711 W. Algonquin Road, Mount Prospect, IL. If one of these items is returned in a book drop, I understand and agree that I am responsible for any and all damages arising from depositing the item in the book drop.

BY SIGNING THIS LIBRARY OF THINGS BORROWING AGREEMENT I ACKNOWLEDGE THAT I AM AT LEAST 18 YEARS OF AGE AND I HAVE READ THIS AGREEMENT AND TAKE RESPONSIBILITY FOR LOSS OR DAMAGE OF ANY EQUIPMENT CONTAINED IN ANY LIBRARY OF THINGS ITEM OR KIT CHECKED OUT ON ANY CARD l LISTED ON THIS AGREEMENT. I AM RESPONSIBLE FOR ALL FEES THAT MAY INCUR IF ITEMS OR ANY CONTENTS ARE LOST OR DAMAGED.
