Marketplace Application Checklist

  • Social Security Numbers (or document numbers for legal immigrants)
  • Employer and income information for every member of your household who needs coverage (e.g. from pay stubs or W-2 forms)
  • Policy numbers for any current health insurance plans covering members of your household
  • A completed Employer Coverage Tool for every job-based plan you or someone in your household is eligible for. (You’ll need to fill out this form even for coverage you’re eligible for but don’t enroll in.)
  • Medications you are taking/Preferred doctors/Preferred hospitals

(Formerly CEDA Northwest) in coordination with the Illinois Department of Public Health and Illinois Health Insurance Marketplace will be enrolling Illinois residents in new coverage options and affordability programs made available through the Federal Affordable Care Act (ACA), beginning October 1, 2013.

Residents served will be those without health insurance and/ or expected to benefit from the ACA’s expansion of the Medicaid program and provision of financial assistance for the purchase of Qualified Health Plans on the newly created Health Insurance Marketplace.
